Can I digitally fill out a fax form? |
Yes, you can easily do so by clicking on each field on the fax form! We recommend digitally completing fax forms (i.e. on your computer) because it allows your information to be captured as accurately as possible. Handwritten forms are accepted, but can be subject to reading errors depending on the quality of the handwriting. Please note: If completing a fax form digitally, use Adobe Acrobat. Some field logics may not function properly in other PDF reader apps, such as Preview. |
Helpful Tips when completing a fax form:
- Cohere's fax form is updated intermittently, please ensure you are using the most up-to-date form by downloading it each time you complete a fax form.
- The “additional care participant” field refers to organizations such as drug suppliers, DME vendors, and clinics. It is only applicable to HealthPartners.
- To quickly fill out the form digitally, use the tab key on your keyboard to move from one field to the next!
